American Heart Association 2021 Recognition Awards

The American Heart Association's Target: Type 2 Diabetes™ Program recognizes North East Medical Services for our commitment to improving quality of care for patients with type 2 diabetes and cardiovascular risk factors.

Furthermore, the American Health Association in association with the American Medical Association recognize North East Medical Services for our commitment to helping improve blood pressure (BP) control rates among our patients, earning Silver level recognition as part of the Target: BP™ Recognition Program.

NEMS Receives 2021 HRSA Community Health Center Quality Recognition Badges

NEMS was recognized by our federal funder, the Health Resources and Services Administration (HRSA), for our excellent quality of care through the Fiscal Year 2021 Community Health Center Quality Recognition (CHQR) Badges. The CHQR badges recognize Health Center Program awardees and look-alikes that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent UDS reporting period. In addition, COVID-19 badges were introduced to recognize health centers’ contributions to the public health emergency response in the areas of data collection, testing, and vaccinations.

NEMS received the Health Center Quality Leaders (Silver), Advancing Health Information Technology (HIT) for Quality, COVID-19 Vaccinations, and Patient Centered Medical Home (PCMH) badges. As a Health Center Quality Leader, we are ranked in the top 11-20% (“Silver” tier) for best overall clinical quality measure (CQM) performance among all health centers. The Advancing HIT for Quality badge recognizes health centers that meet all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care. We received the COVID-19 Vaccinations badge because we have administered vaccines to more than 70% of our 2020 reported UDS patient population. Lastly, we received the PCMH badge in recognition of our nine PCMH clinics.

Patient Centered Medical Home, Certificate of Recognition Level 3
Nine of NEMS’ twelve clinics have been recognized by the National Committee for Quality Assurance (NCQA) as Patient Centered Medical Homes since 2013, achieving Level 3 status in 2014. Level 3 is the highest level of recognition possible for patient-centeredness, care management, and continuous quality improvement.

Quality Improvement Award
NEMS has received Quality Improvement Awards from the federal Health Resources and Services Administration (HRSA) annually since 2014. As a Health Center Quality Leader, NEMS has placed in the top 30% of the adjusted quartile rankings for Clinical Quality Measures among federally qualified health centers across the nation.

Northern California Achievement Award, 80% by 2018 Colorectal Cancer Screening Rate
NEMS’ high colorectal cancer screening rates were acknowledged by the American Cancer Society at California’s Fourth Annual Colorectal Cancer 80% by 2018 Roundtable.

Latent Tuberculosis Infection, Certificate of Recognition
The State of California Department of Public Health, Center for Infectious Diseases presented NEMS with a Certificate of Recognition in 2018 for substantial efforts in promoting the testing and treatment of latent tuberculosis infection.

Public Health Hero, Dr. Jerry Jew
The San Francisco Department of Public Health has recognized Dr. Jerry Jew, MSO Medical Director, for his dedication, passion and leadership in Tuberculosis Elimination.

Hep B United Champion Award
NEMS' efforts to eradicated hepatitis B in Asian and Pacific Islander communities were recognized by the Centers for Disease Control (CDC) Know Hepatitis Campaign and Hep B United in 2013.

Excellence Awards, San Francisco Health Plan
Since 2005, NEMS has received multiple awards for excellence for our role in the San Francisco Health Plan. These include: Award for Excellence in Quality Care (2005); Platinum Award for Excellence (2006-2007); Excellence in Health Technology (2010); Excellence in Customer Service (2012).
