What is the CAHPS Survey?

  • CAHPS stands for: Consumer Assessment of Healthcare Providers and Systems
  • The CAHPS survey asks patients about their care experiences
  • CAHPS survey questions are standardized and multiple choice

  • Make sure all staff members do their part
  • Follow the best examples to improve your experience

How to Complete the CAHPS Survey

Here are some steps to complete the CAHPS Survey, which you will receive after your appointment:

  1. You may receive a survey:
      • Via text within 24 hours
      • Via e-mail within 24 hours
    • Via mail within 1 week
  2. Follow the instructions on the survey form.
  3. Express your thoughts. Please answer all survey questions.
  4. For online surveys, make sure to click on “Submit.” For paper/mail surveys, please return using the envelope provided.
    • No postage or stamps are necessary using the envelope provided.
