How to Complete the CAHPS Survey
Here are some steps to complete the CAHPS Survey, which you will receive after your appointment:
- You may receive a survey:

- Via text within 24 hours
- Via e-mail within 24 hours
- Via mail within 1 week

- Follow the instructions on the survey form.
- Express your thoughts. Please answer all survey questions.
- For online surveys, make sure to click on “Submit.” For paper/mail surveys, please return using the envelope provided.
- No postage or stamps are necessary using the envelope provided.
